Introduction: atherosclerosis represents one of the pathological conditions with the highest impact on human health. In fact, pathologies related to atherosclerosis such as myocardial infarction and stroke are among the main causes of death in nowadays population. In relationship with the enormous importance of atherosclerosis in human pathology, we decided to investigate the history of its pathogenesis through the various historical periods. In particular, our attention was focused on the period between the late nineteenth and early twentieth centuries since in this period medical specializations and the first studies of still current value such as those on the relationship between cholesterol and atherosclerosis developed. Aim of the study: outline the historical evolution of knowledge on the pathogenesis of atherosclerosis. Materials and methods: use of coeval sources and not yet studied together with some revisions on the subject. Results: atherosclerosis is a common pathological condition in populations of the past as well as in current ones. The flourishing of studies on atherosclerosis dates back to the early nineteenth century. Great attention was paid to this pathology between the end of the nineteenth century and the beginning of the twentieth century, when the first experimental studies were also developed. In particular, those on cholesterol, diet and the development of atherosclerotic lesions. The theory of degeneration, encrustation and inflammation are developed. In the second half of the twentieth century, studies on lipids continued, attention was also paid to the cell types of atherosclerosis, endothelial injury and again on inflammatory processes. Conclusions: atherosclerosis represents one of the most complex diseases in the field of human pathology. The approach for its understanding has been and must be multidisciplinary and translational.
